Invitation GezondLeven OpgroeiCheck – around the age of 5

Dear parent(s)/guardian(s),

Youth Health Care (JGZ) from the Municipal Health Service (GGD) invites you for a health check for your child around the age of 5 years old. This is called the GezondLeven OpgroeiCheck.

You fill in an online questionnaire about your child. This helps you see how your child is growing and developing. You can choose topics that are important to you. You can also ask questions and get tips and information.

After you finish the questionnaire, we will invite you and your child for an appointment at a GGD location near you.

How do you complete the OpgroeiCheck?
Please complete the questionnaire within 3 weeks via opgroeicheck.nl. You do not have to finish it in one go.

1. Use your personal login code (you received this in the email)
2. Enter your child’s date of birth (DD-MM-YYYY)
3. Then click on “LOG IN”

Please note: you can use the login code only once. After you log in, you must create an account. With this account, you can look at your answers again later.

Appointment at the GGD
After you complete the questionnaire, you will receive an invitation.
During the appointment:

• we discuss your answers
• we measure and weigh your child
• we check your child’s eyes and hearing

The appointment takes place at a GGD location in your municipality.

Privacy
Your information is private.
We keep it in your child’s medical record. Only professionals involved in care can see this information.

Dear parent(s)/guardian(s),

Youth Health Care (JGZ) from the Municipal Health Service (GGD) invites you for a health check for your child in the 7th grade. This is called the GezondLeven OpgroeiCheck.

You fill in an online questionnaire. This helps you see how your child is doing. For example, how your child grows and develops.

Permission

We need your permission for two things:

• your child to complete the OpgroeiCheck
• us to measure and weigh your child at school

Please answer at least the first 3 questions about permission.

Do this within 3 weeks at:
opgroeicheck.nl

Please do this even if you do not give permission.

How do you log in?

1. Use your personal login code (from the email)

There is a questionnaire for you and one for your child.

You can:

• choose topics that are important to you
• ask questions
• get tips and information

Your child completes their questionnaire at school.

What happens next?

After the questionnaire:

• we measure and weigh your child in a private room at school

Sometimes we want to talk more about the answers. In that case:

• you and your child will receive an invitation
• the appointment will be at a GGD location near you
• you will receive the invitation by email

Sometimes an appointment is not needed. Then you will get a message via:
mijnjgz.ggdru.nl.

You can also update your contact details there.
